The TAF is a process for delivering integrated services for children and young people with additional needs at Tier 2.
The TAF process works with young people up to the age of 25. Not all young people have parents/carers who support them, so the process for them is called ‘Team Around the Young Person’ (TAYP).
At the TAF/ TAYP meeting the family/ Young Person and the TAF team will together agree a plan. The Family/ Young Person and TAF members will review the plan together at TAF review meetings until the Family/ Young Person are happy that they no longer need the support.
The TAF model means that everyone knows who is involved with the family, and it reduces overlap and inconsistency in the services received.
Family/ Young Person Contact.
The Family and Young Person will have a say about who will act as their Family or Young Person Contact. The Family/ Young Person Contact will be the main link for the family/ Young Person and the rest of your TAF and will chair TAF meetings.
